By submitting this form you the buyer acknowledge the following:

  • You have previously set up a North Star customer profile within the store located @ 227 West Avenue Albion, NY 14411, and currently have a valid NYS Drivers License, phone number and email address on file.

  • Before submitting this form you must place and pay for an order of ammunition. Either doing so online, in person, or over the phone. When available add the representative assisting you to our special notes section.

  • Stop in and see us anytime, we love helping customers get approved for all of their items. Sometimes background checks are delayed. Once a final decision has been reached, we will contact you. Thank you for your patience.

The individual purchasing ammunition and filling out the form must be the same person who picks it up at the store, and they must present valid identification.
